Quality score and repeat errors

Objective. Equip Sales and CSM to run the quality play: the customer's quality score looks acceptable yet the same errors keep recurring and rework stays high. Show how Quality and Call QA, Work Output and Work Understanding turn evaluation into root cause and closed-loop coaching, and keep the coverage-is-not-accuracy misread out of the story.

A healthy quality score is often an illusion of control: it can be high while the same defects repeat, because coverage and scoring consistency are not the same as fixing the cause. This play chains evaluation to improvement: Quality and Call QA runs evaluation, calibration, disputes and coaching; Work Output shows the quality and rework cost of the output itself; Work Understanding finds the process root cause behind recurring errors. The value is fewer repeat errors and less rework, not a prettier score. The boundary: high evaluation coverage does not prove scoring accuracy, AI evaluation needs human review, and a score is a signal, not a fixed cause.

The problem and who feels it

The quality dashboard says things are fine - the average score is green - yet the same mistakes keep coming back, rework stays high, and customers or payers keep flagging the same defect type. Who feels it: a quality or operations leader whose score looks good but whose rework and complaint volumes say otherwise; a team lead coaching the same errors month after month with no durable change; a CSM or buyer who suspects the score is measuring activity (evaluations done) rather than improvement (errors prevented).

The deeper problem is that an evaluation program can be busy and consistent-looking while never closing the loop: errors are scored but not traced to a cause, coaching is logged but not tied to a recurring defect, and no one can say whether the program is actually reducing errors.

The business impact

Quality programs that do not close the loop cost money while looking healthy. Rework and waste - the same defect reworked repeatedly consumes capacity that a fix would free. Downstream failure - in RCM a recurring documentation or coding error becomes denials and aged AR; in a contact centre it becomes repeat contacts and churn. Wasted evaluation effort - reviewers spend hours scoring without the calibration, root cause and coaching that would turn scores into prevention. And false confidence - leaders report a good score to their own stakeholders while the structural error persists, so the correction is delayed.

The decision this play protects is whether to trust the score: a high number that hides repeat errors leads to under-investment in the fix.

Which applications address it and how they chain

The play chains evaluation to cause to prevention.

  1. Quality and Call QA (https://quality.demo.timechamp.io/) - the Command Center runs evaluation against rubrics, sampling, calibration, four-eye checks, disputes, root cause and coaching, with AI output positioned alongside human review. This is where scores become consistent and defensible and where coaching is logged against the defect.
  2. Work Output (https://work-output.demo.timechamp.io/) - shows the quality and rework cost of the actual output, so a quality score connects to clean output per paid hour and the cost of defects - quality framed as money, not just a rubric number.
  3. Work Understanding (https://work-understanding.demo.timechamp.io/) - finds the process root cause behind a recurring error (a friction, a missing step, a variant), so the fix is structural rather than repeated coaching of a symptom.

The chain: consistent, calibrated evaluation and coaching (Quality and Call QA) - the rework and cost the defects create (Work Output) - the process cause that must change to stop them (Work Understanding). Together they turn a score into a prevented error.

The value story and how it is measured

The value is fewer repeat errors and less rework, driven by a program that traces defects to a cause and closes the coaching loop - not a program that simply reports a score.

Measure against the customer's baseline with the applications' own measures: calibration agreement and review turnaround (is scoring consistent and timely), dispute resolution, recurring errors and coaching effectiveness from Quality and Call QA; quality and rework cost from Work Output; and measured time released once a Work Understanding improvement is validated before/after. The headline score is demoted from the goal to one input - the real goal is the recurring-error trend going down with evidence.

The honest boundary - what this does not solve

This play makes quality measurable and traceable; it does not guarantee a better score or replace human judgement. Be explicit.

  • Coverage is not accuracy: high evaluation coverage alone does not prove the scoring is accurate or that quality is improving. Volume of evaluations is an activity measure, not an outcome.
  • AI evaluation needs human review: AI-assisted scoring and root-cause suggestions are candidates, positioned with human review and evidence - never presented as final judgement or proven causality.
  • A score is a signal, not a cause: a low score tells you where to look, not why. The cause comes from inspecting evidence and, for recurring errors, from Work Understanding - not from the number itself.
  • Improvements must be measured, not assumed: a coaching action or process change is a hypothesis until before/after evidence confirms the recurring error actually fell, and overlapping fixes cannot be summed into one savings claim.

Interpretation check

A healthy quality score is not proof of quality. High evaluation coverage does not prove scoring accuracy or improvement, a score is a signal rather than a cause, and AI-assisted evaluation is a candidate for human review - the real outcome is the recurring-error and rework trend falling with before/after evidence.

In the field

OVO contact-centre and KnackGlobal RCM both show the pattern: a healthy QA score alongside persistent repeat contacts or recurring documentation/coding defects that become denials. The play traces the defect to a cause, ties coaching to it, quantifies the rework in Work Output, and proves the recurring error fell with before/after evidence - rather than celebrating a score that never moved the real problem.

Practical task

In the demo, build the quality story for one recurring error: show consistent, calibrated evaluation and coaching in Quality and Call QA, the rework cost in Work Output, and the process root cause in Work Understanding. Write the one sentence that retires the 'high score means good quality' misread, and state how you would position AI-assisted evaluation honestly to the buyer.

Self-check

A customer's average quality score is 92% but the same error keeps recurring. What is the misread and the correct read?

The misread is treating a high average score as proof of quality. The correct read: coverage and a good average do not prevent errors. Trace the recurring defect to a cause (Work Understanding), tie coaching to it (Quality and Call QA), and watch the recurring-error trend, not the headline score.

Why bring Work Output into a quality conversation?

To frame quality as money. Work Output shows the rework cost and the hit to clean output per paid hour that defects cause, so quality becomes a business figure (cost of defects) rather than an abstract rubric score.

How should AI-generated evaluations be positioned to a buyer?

As candidates that accelerate and widen coverage but are reviewed by humans with evidence - calibration, four-eye checks and disputes exist exactly so AI output is validated, not trusted blindly. Never present AI scoring as final or as proven causality.

What makes a coaching action in this play credible rather than box-ticking?

It is tied to a specific recurring defect traced to a cause, and its effect is measured before/after - the recurring error actually falls. A logged coaching note with no cause and no measured change is activity, not improvement.

The customer asks you to add up savings from three quality fixes. What is the caution?

Overlapping findings and fixes cannot simply be summed - they may address the same rework. Validate each independently and avoid a combined savings claim that double-counts.

Verified vs demo. The evaluation, calibration, dispute and coaching flow, the rework-cost view and the root-cause discovery are demo-shown on seeded data. The customer's actual rubric, sampling, reviewer calibration, AI configuration and the real recurring-error trend must be validated with product and measured in the account; root causes are candidates to confirm, not proven.
